If you choose Personalized, please include a single-column
list of the names (no titles, departments, etc.) in both
an electronic format (MS Word or Excel preferred) and a hard copy. No name
can be longer than 25 characters.
- Determine your service charge.
The service charge per certificate is $0.75 for non-personalized
certificates and $0.90 for personalized certificates.
Note, for orders of less than 20 certificates the service charge is $15.00

Certificates will be sent by insured registered mail to one
location. Drop shipments to multiple locations are available at extra cost.
At your request we will ship via Fed Ex (using your account number). but cannot
assume any liability as express carriers do not insure at full value.
